or other means) and provide a confirmation, or when you receive a clear confirmation message from

You agree to provide accurate and complete information when submitting an appointment request and to
update us promptly if any information changes. We are not responsible for issues arising from
incorrect or incomplete information provided by you.

We reserve the right to cancel or reschedule appointments due to doctor availability, emergencies,
operational requirements, public health considerations, or other reasons. We may assign a different
doctor from the one requested, where necessary.
